GETTING STARTED: There is a US$ 97.00 license application and processing fee. For security and economic reasons we do not accept credit cards on or off line. You may either read, complete and submit the license agreement and application electronically (we will send you an acknowledgment and invoice) OR you may print and complete this form, attach the requisite check (made payable to Dr Sylvia Marnella, LLC) and mail it to: MPDCS, 10250 Camino Santa Fe, San Diego, CA 92121-3105. Please indicate the "user name" you would like to use to log on to the site (on the application below). We will acknowledge your application and send you a password. Both will be needed to access the complete site and examination sessions.
Examination sessions are pre-purchased in blocks of 5 (US $245 = $49.00/each) or 10 (US $390 = $39.00/each). The computer will automatically keep track and inform you of the number of unused/prepaid sessions. To order examination sessions, please complete the Order Examination Sessions below. You may submit this form electronically (we will send you and acknowledgment and invoice) OR you may print and complete this form, attach the requisite check (made payable to Dr Sylvia Marnella, LLC) and mail it to MPDCS, 10250 Camino Santa Fe, San Diego, CA 92121-3105. LICENSE AGREEMENT AND APPLICATION
This application and agreement between Dr Sylvia Marnella LLC (Seller) and the below named Licensee, for the use of this scale, signifies the applicant/user has read, understood and agrees to these terms and conditions. Use of this Website and the Marnella Parent Donor Comparison Scale is limited to Physicians, Psychologists and certain other professionals working in the field of Assisted Human Reproduction. All materials are copyrighted. All rights are reserved. Their reproduction, adaptation, or recording in whole or in part for any purpose or by any process or means, without prior written permission from the author is prohibited. It is understood that this scale is NOT a psychological "test" by traditional definition and should NOT be used for diagnostic purposes. It is NOT intended to replace mental health screens or suitability testing. It is also NOT a predictor of outcome. It's sole purpose is as a comparative tool for the improved matching of Prospective Parent and Sperm or Oocyte Donors. The issue of application and limitations are the responsibility of, and should be explained by the Licensee to the respective Candidate Parents and Donors. The Seller's sole warrant is proper Website function and the timely delivery of scale results. The Licensee's sole remedy will be, at the Seller's option, either a refund or additional scale "sessions". Seller shall not be liable for any indirect, incidental, or consequential loss or damage of any kind whether resulting from any breach of the warranty or resulting from any actions by the Licensee.
